Instructions:
This form shall be prepared by the various department/office/unit heads to present their annual/supplemental office
supplies requirement for the calendar year.  It shall serve as a supporting document for the comprehensive release of
MOOE items falling under the Needing Clearance portion of the LBM.
The same shall be submitted to the Bids and Award Committee for recommendation, and to the local budget officer
for review. It shall be approved by the local chief executive.
